Enrollment and Withdrawal Procedures

 

Enrollment Requirements for Kalakaua Middle

 

Students transferring from another Hawaii Department of Education (DOE) School:

  1. Parent/Guardian with valid picture ID (driver’s license, state ID, passport)
  2. Valid adoption papers/Power of Attorney (if applicable)
  3. School release packet from previous school (including immunization record/TB clearance and proof of physical exam)
  4. Registration forms completely filled out (given by Kalakaua Middle School)
  5. TWO of the following items that show the parent/guardian’s name and address within our district (current/valid: rental/lease agreement, mortgage payment, property tax bill, state ID, driver’s license, electric bill, gas bill, water bill, cable bill, phone bill)

 

Students transferring from a United States non-DOE school:

  1. All of the above
  2. Student’s birth certificate

 

Students transferring from a foreign country:

  1. All of the above
  2. Student’s passport

 

Withdrawal Requirements for Kalakaua Middle

  1. At least two school days before the child’s last day of school, please notify Kalakaua Middle School’s registration office of your intent to withdraw the child (please call 305-4531 or 305-4550).
  2. Parent/Guardian will bring a valid picture ID (driver’s license, state ID, passport) to Kalakaua Middle School’s registration office.
  3. Parent/Guardian will fill out the Request for Release document.
  4. Parent/Guardian will be given a release packet to take to the next school.
